I hereby make an application to join the Construction, Forestry, Maritime, Mining & Energy Union, Construction & General Division, ACT Divisional Branch. If admitted, I agree to compy with the Rules of the Union. By joining the Union I have appointed the Union as my representative for all purposes relating to my employment, including representation in industrial disputes and in enterprise bargaining in accordance with 5.176 of the Fair Work Act 2009.
